• Fall Sports Participation Information

    If your 7th or 8th grade student is interested in fall sports, please register them in Final Forms. Be sure to select any and all sports your athlete may be interested throughout the course of the school year. Coaches will use Final Forms to communicate with parents regarding preseason, try-outs and practices.  Instructions for Final Forms can be found on the Athletic website.

    IMPORTANT! All physical forms need to be turned in by July 26th. Physicals can be turned into the office the week of July 22-26th or uploaded to Final Forms

    The official season for all fall sports will begin August 1, 2024. Tryouts will also begin on August 1.

    MyPaymentsPlus (MPP): Pay Athletic Fees--Fees will be posted to MPP accounts as soon as rosters are finalized by the coaches. 

    • Athletic fees must be paid prior to the first game.
    • Participation Fee for all Sports: $125.00 per sport. Reduced lunch: $62.50. Free lunch: No Fee.

    FINAL FORMS: All required athletic forms and documents are online at the FinalForms website.  Parents AND students must complete all the information to participate in a sport.

    2024-25 OHSAA Preparticipation Physical Evaluation form:  Student athletes must have a CURRENT OHSAA Preparticipation Physical Evaluation form on file in the Perry Athletic office to participate in any sport. ONLY Pages 4 & 5 of the OHSAA Preparticipation Physical Evaluation Form need to be turned in. All other information is to be completed on your Final Forms account (see link above).
