2023 Cardinal Middle School Spring Sports Information

Specific Spring Sports are a combination of students who attend Kilbourne, Phoenix and Worthingway Middle School.   

Final Forms and Sports Physical completion:  ALL athletes / parents (grades 7-12) use FinalForms. Please create an account https://worthington-oh.finalforms.com and electronically complete all forms - both parent and student must complete them. Please check the spring sport you are participating in so you are added to the roster. All FinalForms must be completed and a sports physical (https://ohsaaweb.blob.core.windows.net/files/Sports-Medicine/2022-23PPE(English).pdf) completed by a physician (valid for one calendar year) must be on file prior to the first practice date.

Pay-to-Play Fee: There is a participation fee for sports. The Spring Sport fee is $125.00 and will be posted to the student's MyPaymentsPlus account once the team rosters are finalized. If you qualify for free & reduced fees this will be applicable.

Shuttle Bus Information: There will be shuttle buses to transport all students to after-school practice sites. Parents will pick students up at the practice site.
