• REQUIREMENTS TO PARTICIPATE IN MIDDLE SCHOOL SPORTS 

    1. ONLINE FINAL FORMS ACCOUNT SET-UP AND FORM SIGNATURES COMPLETION

    Worthington Schools collects athletic forms online through Final Forms. A parent/guardian must set-up an online account and complete all required form signatures through their online account. Click for direct links to  Final Forms and Final Forms Parent Directions. Once an account is established and the parent/guardian form signatures are complete, the registered student(s) will receive an email with a link to complete their required form signatures. Please note: The only paper form copies accepted in the school office are page 4 & 5 of the OHSAA Preparticipation Physical Evaluation Form > see more information on the physical below. 

    STUDENTS WILL NOT BE ABLE TO PARTICIPATE IF THE STUDENT IS NOT COMPLETELY REGISTERED ONLINE THROUGH FINAL FORMS > THIS INCLUDES PARTICIPATION IN TRYOUTS, IF APPLICABLE TO THE SPORT. 

    2. COMPLETED OHSAA PREPARTICIPATION PHYSICAL EVALUATION FORM

    A current OHSAA Preparticipation Physical Evaluation Form, signed/stamped/dated by a physician, must be on file in the school office or uploaded to the students Final Forms account. ONLY pages 4 & 5 > Medical Eligibility Form and Physical Examination Form (which are signed/stamped/dated by a physician) need to be turned into the school office or uploaded to the students Final Forms account. All other pages/form signatures are to be completed online through the Final Forms account. OHSAA Physicals are good for 13 months from the exam date. Physicals MUST BE the OHSAA Preparticipation Physical Evaluation Form > no other forms, letters, etc. will be accepted.     STUDENTS WILL NOT BE ABLE TO PARTICIPATE IF A CURRENT OHSAA PREPARTICIPATION PHYSICAL EVALUATION FORM IS NOT ON FILE > THIS INCLUDES PARTICIPATION IN TRYOUTS, IF APPLICABLE TO THE SPORT. 

    3. PAYMENT OF THE DISTRICT PAY-TO-PLAY FEE

    There is a pay-to-play fee for each sport, each season. The fee for all sports, except cheerleading, is $125.00 per sport. The cheerleading fee is $85 per season, as well as a $5 catastrophic insurance fee, which is paid once annually. Fees will be posted/paid online through MyPaymentsPlus > if you don’t have an account set-up, please contact the WWay office (614-450-4300) for your student's Student Number in order to create an online account. Once the Worthingway office receives the final rosters from coaches, the fees will be uploaded to your MyPaymentsPlus account. The coaches will inform parents/guardians when this has taken place. The fee must be paid online before the first contest. Athletes will not be able to participate in the first contest until fees are paid. If you have district Free/Reduced approval and you completed the “Sharing Information With Other Programs” form in the Free/Reduced application, then your pay-to-play fee will reflect either free or reduced. Please note: The catastrophic insurance fee for cheerleading does not qualify for free/reduced.

    STUDENTS WILL NOT BE ABLE TO PARTICIPATE IF THE PAY-TO-PLAY FEE / CATASTROPHIC INSURANCE FEE FOR CHEERLEADING IS NOT PAID BEFORE THE FIRST CONTEST.

    2024 SEASON COACHES

    2024 SEASON INFORMATION

    • Due to low numbers, KMS will be merging with us for this season. This will increase the numbers of athletes coming to open gyms and those trying out. To put yourself in the best position, please make sure your Final Forms documents are current ASAP! If your forms are not current, you will not be allowed to try out! 

    • We are hosting open gyms at Worthingway on Saturday, January 13th and Saturday, January 20th from 2:30-4:30pm.

    • Our February open gyms are Thursday the 15th and Friday the 16th from 4:15 - 6:00pm

    • One of the growing pains of our program is learning to work with two different school calendars. :) We have discovered conflicts with both the KMS and WWAY schedules (including one competely unrelated to sports) that are out of our control. This will force us to make three (3) changes to our calendar for Tryouts, our first practice, and our parents meeting. The changes are as follows:

      (1) Tryouts are now Wednesday, February 21st and Thursday, February 22nd from 4:15-6pm at WWAY. KMS athetes will be shuttled over to WWAY from KMS.

      (2) Our first practice on Friday, February 23rd from 4:15-6pm has been moved to KMS. WWAY athetes will be shuttled to KMS from WWAY.

      (3) Due to our Friday, February 23rd practice changing venues, our parents meeting is now in the KMS library immediately after practice.  

      We hope these types of changes will be a thing of the past as we work closely with administration from both schools this season. Thank you for understanding and being patient with us as we navigate these uncharted waters. Having a popular program that continues to grow and bust at the seams is a good problem to have! Thank you for being awesome!

    • The parent meeting will be beneficial for several reasons including setting up volunteers for:
      • team photographer
      • keeping the book
      • line judges
      • tickets
      • concessions.
    • Also at the parent meeting we'll be announcing:
      • team captains
      • introducing this years assistant coaches
      • Introducing new student managers and helpers for what is going to be a very special season! Our student involvement continues to grow each year and we love it!

    • We will have 2 teams, an A/B team.

    • Canterbury will coach A and Minarchek will coach B.
