Immunization Reminder for Parents of 7th and 12th Graders:
The Ohio Department of Health requires all 7th graders to have a Tdap vaccine and a Meningococcal vaccine prior to the start of school. All 12th graders are required to have 2 doses of the Meningococcal vaccine (only one dose is required if it is received on or after the 16th birthday).Children will not be permitted to start school without proper proof of these immunizations. It is advisable to make an appointment for your child to receive these vaccines to avoid exclusion. As soon as your child receives this vaccine, please provide proof of immunization from your physician to your school office so your child’s school health record can be updated.If you are having difficulty obtaining the Tdap or Meningococcal vaccines, please see the clinic opportunities listed below.
